Who May Need PGT? Indications and Counseling Points for Embryo Genetic Testing
Whether PGT is appropriate depends on age, medical history, family genetic risk, embryo number, local regulation and genetic counseling. It should not be presented as a default test for all IVF patients.
PGT is a group of genetic testing tools that may be used in IVF cycles. It can help selected patients understand certain chromosomal or genetic risks in embryos, but it is not necessary for every IVF patient. Whether PGT is appropriate should depend on medical indication, family history, embryo number, patient goals and informed counseling.
Many patients ask whether advanced IVF or PGT is always better. The answer depends on the problem being addressed. Without a clear genetic or clinical reason, adding testing may increase cost, waiting time and emotional burden without clear benefit.
Patients of advanced reproductive age may discuss PGT-A because chromosomal abnormality rates generally increase with age. However, PGT-A cannot guarantee implantation or pregnancy and does not reverse age-related egg quality changes. The decision should consider embryo number, previous treatment history, pregnancy loss history, cost and counseling.
Patients with recurrent pregnancy loss or repeated implantation failure may also discuss PGT-A, but repeated failure may involve many factors, including embryo development, uterine conditions, endocrine issues, sperm factors, laboratory records and transfer technique. PGT may provide information but should not be treated as the only solution.
Families with known monogenic disease risk may consider PGT-M. This usually requires genetic counseling, confirmation of the familial variant, family analysis and test development. PGT-M is targeted testing for known risk, not a broad disease screen.
Patients or couples with chromosomal structural rearrangements may consider PGT-SR. This may apply to balanced translocations, inversions or other structural rearrangements. Genetic counseling and karyotype information are important before testing.
Pre-test counseling is essential for all PGT categories. It helps patients understand the purpose, limitations, possible results, ethical issues, cost, waiting time and next steps.
