Male patient diagnosis: Congenital absence of the vas deferens—a structural anomaly preventing natural sperm egress. Intervention: Testicular sperm aspiration (TESA) to retrieve sperm directly from testicular tissue. Sperm processing: Two advanced cryopreservation techniques applied due to severe oligospermia—7 straws for low-sperm-volume freezing and 40 individually frozen spermatozoa for ICSI (intracytoplasmic sperm injection).
Male Patient Clinical Summary
Diagnosis:
Congenital absence of the vas deferens—an anatomical anomaly resulting in failure of sperm egress via natural pathways.
Intervention:
Testicular sperm aspiration (TESA) was performed to directly retrieve sperm from testicular tissue.
Sperm Processing:
Due to extremely low sperm count, two highly efficient sperm cryopreservation techniques were employed.
Low-volume sperm cryopreservation: 7 straws stored.
Single-sperm cryopreservation: 40 individual spermatozoa preserved for ICSI (intracytoplasmic sperm injection).

Female Patient Clinical Summary
Age: 38 years
Ovarian Function:
Baseline hormonal testing (FSH, LH, AMH) indicated adequate ovarian reserve, though age-related decline remains a concern.
Transvaginal ultrasound revealed a moderate antral follicle count (AFC), suggesting favorable responsiveness to ovarian stimulation.
Ovarian Stimulation Protocol:
A personalized Progesterone-Primed Ovarian Stimulation (PPOS) protocol was implemented to maximize oocyte quality while minimizing the risk of OHSS (ovarian hyperstimulation syndrome).
Oocyte Retrieval Outcome:
A total of 21 oocytes were retrieved, including 15 mature MII oocytes and 6 immature (GV/MI) oocytes.
All 15 mature oocytes underwent ICSI fertilization.

Fertilization and Blastocyst Culture:
Of the 15 mature oocytes subjected to ICSI, 13 exhibited normal fertilization (2PN), while 2 displayed 1PN formation.

The 6 immature oocytes underwent in vitro maturation (IVM); 5 successfully matured, all 5 fertilized (100% fertilization rate), and yielded 3 Day-5 blastocysts.
Ultimately, a total of 14 blastocysts were successfully cultured!
