Introduction
Today, we are delighted to share a warm and inspiring case: a 39-year-old woman and her 42-year-old partner successfully achieved pregnancy through third-generation IVF (PGT-A). This case vividly illustrates how personalized medical protocols play a pivotal role in helping couples realize their dream of parenthood—like a magical key unlocking the door to a joyful family life.
Basic Information
Consulting Physician: Dr. Liu
Consultation Period: June 24–29, 2024
Medical Plan
Under the meticulous guidance of Dr. Liu—a highly skilled and experienced fertility specialist—the couple underwent a comprehensive and thorough evaluation. Based on their individual health profiles and fertility goals, Dr. Liu designed a detailed, customized IVF treatment plan—akin to crafting a precise battle strategy for a critical mission—laying a solid foundation for all subsequent treatment steps.

Treatment Protocol
Ovarian Stimulation & Monitoring: Ovarian stimulation was performed using Gonal-F®, Cetrotide®, and Clomiphene Citrate. Concurrently, regular hormonal assays and transvaginal ultrasound monitoring served as a precise navigation system—tracking synchronized follicular development and maturation in real time to ensure seamless progression at every stage.
Egg Retrieval: Once follicles reached optimal maturity, eggs were retrieved via minimally invasive surgery—carefully and gently—preparing the way for subsequent fertilization, much like preparing the most beautiful bride for her grand wedding day.
Fertilization Procedure: Retrieved oocytes were combined with sperm in vitro. Depending on clinical indications, either conventional IVF or ICSI (intracytoplasmic sperm injection) was selected to maximize fertilization rates—like a master artisan meticulously shaping each embryo’s potential for life.
Embryo Culture & PGT-A Screening: Fertilized oocytes were cultured to the blastocyst stage under strict laboratory conditions, followed by preimplantation genetic testing for aneuploidy (PGT-A). This rigorous screening acted like a quality-control inspector—identifying chromosomally normal, healthy embryos and ensuring the very first safeguard for the baby’s lifelong well-being.
Embryo Transfer & Post-Transfer Monitoring: A chromosomally normal blastocyst was selected for transfer. Following transfer, serial hormonal assessments and ultrasound examinations were conducted—like a devoted guardian—ensuring successful implantation and healthy embryonic development, and providing steadfast support for the arrival of new life.

Egg Retrieval Outcome
The IVF-ET laboratory report indicated retrieval of 7 oocytes total: 4 mature (MII), 2 immature (GV stage), and 1 abnormal. This outcome fell within expected parameters and provided an excellent foundation for subsequent fertilization and embryo culture—like fertile soil offering ideal conditions for seed germination.
Day-3 Embryo Results
During early embryo culture, 4 cleavage-stage embryos developed: 3 graded as Grade A and 1 as Grade B. All embryos were extended to blastocyst culture, reflecting a collective aspiration—like young saplings striving upward toward full growth and potential.

Day-5 Blastocyst Results
With dedicated care from the clinical and embryology teams, all 3 embryos successfully developed into blastocysts—and all were deemed viable for transfer. Blastocyst development is a critical milestone determining transplant success—like students delivering outstanding results on a pivotal exam.
PGT-A Screening Results
PGT-A analysis confirmed two euploid embryos—one male (XY) and one female (XX). PGT-A is an advanced genetic screening method capable of precisely detecting chromosomal abnormalities in embryos, thereby significantly improving implantation rates and reducing risks of miscarriage and genetic disorders—building a robust protective barrier for lifelong health of the future child.
